What is Hallucination?

When an AI model produces fluent, confident output that is false or not supported by its sources.

Hallucinations happen because models are trained to produce plausible text and don't reliably know what they don't know. Reduce them with RAG, required citations, permission to say "I don't know" and verification in code.
